How do I stop denials from medicare for 99244 with mod 25 and 95834? Is there a max limit on times 95834 b bil

The neurologist performs the 95,834 with every encounter with a patient. I'm not sure if the concerns that are using Modifier, though? dx paid only with specific periods of maximum # pu? be loaded out, or what? A denial by state health care, said 95,834 Procedure Code was not necessary medicines.
